Refer a Friend Program

In this document:

• ‘Referrer’ means an existing patient of a Relevant Practice who refers a friend or family member to the Program.
• ‘Referral’ (or ‘Referred Patient’) means a new patient who has been referred by a Referrer.
• ‘Relevant Practice’ means Port Kennedy Central Dental Care.

Program benefits

• Referral: $30 account credit applied to out-of-pocket expenses.
• Referrer: Entry into prize draw for each successful referral.
• Referrers refer friends and family to receive an entry into our Refer a Friend prize draw. The Referral receives a $30 account credit.

The $30 account credit applies only to the Referral’s out-of-pocket expenses, not their health fund rebate (where applicable). If the Referral’s health fund covers the cost of their first appointment, the $30 credit applies to a later appointment or treatment (excluding treatments involving cosmetic injectables).

Eligibility criteria

• Referrers must be existing patients of a participating Relevant Practice and over the age of 18.
• Referrals visit any Relevant Practice and must also be over the age of 18.
• Referrals must be new patients (not existing patients of any Relevant Practice).

The $30 account credit or prize draw entry is not exchanged for cash. There is no limit to the number of referrals a Referrer makes, and the prize draw entry applies for each successful referral.

Offer period

This Offer is ongoing. Port Kennedy Central Dental Care reserves the right to withdraw it at any time without notice.

Redemption process

• Referrers: Provide your friend or family member with your name to mention when they book their appointment so we verify the referral.
• Referrals: Mention the name of the person who referred you prior to attending your first appointment or present a referral voucher at reception.
• Once a referral has been verified, Referrers will be automatically entered into the prize draw.

Privacy and consent

• You acknowledge and agree that identifying you as a patient constitutes health information, and you consent to Port Kennedy Central Dental Care using this information for Program purposes.
• You consent to having your name published on Port Kennedy Central Dental Care’s website if you win the prize draw.

Offer restrictions

• Only existing patients of Relevant Practices refer friends and family.
• Referrers and Referrals must be over 18 years old.
• The $30 account credit applies only to out-of-pocket expenses.
• The $30 account credit is not used for treatments involving cosmetic injectables.
• Account credit is valid for 12 months from the date of issue. Port Kennedy Central Dental Care extends credit on request.
• This Offer works with other offers, including the new patient offer.
• The Referral must notify the Relevant Practice of their referrer’s details prior to attending their first appointment to claim the discount.
• Referrers accumulate prize draw entries from multiple referrals. There is no limit to the amount of patients each Referrer refers to a Relevant Practice.
